Lower back pain is one of the most common reasons people visit our Melbourne Chiropractic clinic. In this case study, we share how a middle-aged man achieved complete relief from acute mechanical lower back pain in just three sessions.
Patient Profile
JC (full name replaced for confidentiality), a 52-year-old project manager from Melbourne’s inner north, was seeking treatment after experiencing sudden-onset of lower back pain that had started two days earlier. He described acute central lower back pain (around the L4-L5 region) with mild referral into the left gluteal area. He required a lumbar support brace for stability. Difficulty was noted when bending forward and transitioning from sitting to standing. It was interfering with his sleep, work, and daily activities.
Initial Chiropractic Assessment
On examination:
- JC had noticeable stiffness and guarding in the lumbar spine.
- Active lumbar flexion and extension were limited and painful.
- All orthopaedic tests (including straight leg raise, slump test, and lumbar quadrant tests) were negative. Suggesting no disc pathology.
- All neurological tests (reflexes, muscle strength, and sensation) were also normal.
- No red flags were present. Learn more about the importance of identifying red flags here.
This presentation was consistent with acute mechanical lower back pain — a common, non-complicated condition involving joint dysfunction and surrounding soft tissue irritation.
Treatment Approach
Using an evidence-based treatment approach, we provided:
Session 1:
- Gentle soft tissue therapy to release tight lumbar paraspinal muscles and left gluteal muscles (including piriformis and gluteus medius).
- Specific manual lumbar spinal manipulative therapy (SMT) targeted at the dysfunctional segments.
- Post-treatment advice: gentle walking, and simple movement modifications.
JC reported an immediate 40–50% reduction in pain intensity following the first consultation.
Session 2 (5 days later):
- Continued soft tissue therapy with improved tolerance.
- Further lumbar spinal manipulation focusing on restoring segmental mobility.
- Introduction of basic lumbar stabilisation exercises.
By the end of this session, JC’s pain had reduced to a mild level (2/10) and gluteal referral had resolved.
Session 3 (one week after Session 2):
- Final spinal manipulation and soft tissue work.
- Progression of home exercises to include core activation and mobility drills.
Outcome
After three chiropractic sessions, JC’s pain had completely resolved. He regained full lumbar range of motion and was able to return to normal work and recreational activities without discomfort.
What Can We Learn From This Case?
Not every episode of lower back pain follows the same course. Thorough assessment is essential to determine whether conservative care is appropriate and to identify individuals who may require further investigation or referral.
For patients experiencing uncomplicated acute mechanical lower back pain, evidence-informed chiropractic management may help support recovery by:
- Reducing pain.
- Improving movement.
- Restoring function.
- Providing reassurance and education.
- Encouraging an early return to normal activities.
Why This Approach Worked
Acute mechanical lower back pain responds very well to early intervention combining:
- Hands-on manual therapy (spinal manipulation)
- Targeted soft tissue release
- Patient education and appropriate movement
Research consistently supports the use of spinal manipulative therapy for acute low back pain, often leading to faster recovery compared to rest or medication alone.
